Greg Brady is a serial innovator and the visionary behind Connect4Patients. He previously founded One Network Enterprises, where he introduced real-time collaboration and the concept of the single version of the truth to transform global supply chains. His leadership at i2 Technologies and Oracle helped redefine how data powers decision-making at scale.

Now, Greg is applying that same bold thinking to healthcare, connecting patients, providers, caregivers, and systems around one unified experience. His belief is simple but radical: when everyone shares the same information, better outcomes follow.

Jay joined C4P as the Chief Platform Architect, designed the architecture that enables the interoperability required to bring health and wellness data into C4P. He also leads our technology organization and has developed the key components of C4P including the consumer facing application, health & wellness data integration layer and the generative AI (model provider agnostic) workflows and agents leveraged in C4P. Before joining C4P, he served as a Principal Architect at Optum and has over 8 years of experience both building & designing solutions in health care technology.

With deep experience across clinical outcomes, patient satisfaction, and care strategy, Kelly ensures C4P reflects what truly works. She most recently served as System VP of Health Outcomes & CDI at Christus Health and led the national HIIN program at Vizient.

"I'm passionate about understanding lifecare — not just healthcare or prevention, but what truly serves people when, where, and how they need it."

Lorina ensures the company runs with discipline and clarity. She brings cross-industry leadership in operations, compliance, and execution.

Tristen leads brand, go-to-market, and user engagement. He brings experience from One Network Enterprises and a passion for making complex systems feel simple and human.
